When a corrupted profile is not evident the answer may lie in windows itself.

Win7 has a change for text size: 100% 125% 150%
When nothing seemed to work with AutoCAD itself I went to Control Panel|Display and changed from 100% to 125% and that forced log off. After log on and load Tool bars showed correctly.

By going back to 100% Bars swapped backwards again. This time I re-positioned the bars, and did a qsave twice and logged off. After reload all was well.

***Next time this happens, do the above first and see that fixes it.***

I think that I have the same / a similar problem, but I don't understand your information completely. Please could you add some picture for

- what happened before
- what did you do
- what happens after

Please note that this experience is using Win7 and acad2004. My app uses two docked bars at left of screen. The first to go up always stays up. The second displays as needed to the right of it. Quite often visual lisp will lock up and stop responding after an error. This occurs more when certain other application window are maximized behind AutoCAD and Visual lisp. Sometimes toggling Windows button-D will break the the lockup, other times it is necessary to kill the process with task manager. Restarting and loading AutoCAD and OpenDCL project again, will some time restore the docked bars in reverse order. Sometimes over under in same bar space. And rarely completely off of the screen.

Action bar error : Swap Position
Intermittently after a crash, two left side docked control bars will exchange position as in:
Bar1 Bar2 -> Bar2 Bar1. When a corrupted profile is not evident the answer may lie in windows itself.

Win7 has a change for text size: 100% 125% 150%
Go to Control Panel|Display in Win7 and change text size: from 100% to 125% that will force log off.
Load Acad and Visual Lisp. Docked Action bar should now appear on right. Do a acad quick save twice.
Change Win7 Display text size to: 100%
Forces Log off.
Load Acad and Visual Lisp. Docked Action bar should now appear on right as before.

Note: By going back to 100% without the qsave Bars swapped backwards again
